He looks like a lot of middle aged blokes do at that age. Unfortunately because of the line of work he did/does. He can't get away with letting himself go (not that it matters) like Joe Public.
He never looked after himself when playing - cigs, pies and pints every summer. That approach (together with the sheer volume of games he played) meant his legs had gone in his early 30s and he wasn't good enough in a midfield role to carry on at the top level.

It's like the weekends. When you're in your twenties up to the age of about 25 you can get away with drinking Friday while Sunday with Kebabs, Pizza's and Curries. Then unless you exercise like the clappers it just piles on. I should imagine Rooney needed boxing style training camps to get fit.
Listened to a podcast other day and he admitted he never looked after himself like others did and was a huge contributing factor to why he retired so early. He also said he was lucky to get away with it during his career as he was naturally able to hold it back but years took its toll eventually.
He had a fantastic career in spite of the way he looked after himself, rather than because of the way he looked after himself. By way of contrast, Ronaldo is older than he is.
