There's a model used mainly in health and safety. Anyone investigating an accident will typically find 2-3 minor errors that lead to the actual accident. Like layers of Swiss cheese where the holes line up to create one hole a clear route to disaster. We are like the Swiss cheese model. We often, and I mean often, have a series of errors that lead to a goal.

    Vs Altrincham

    Giveaway in midfield
Shot pushed into path of oncoming player
    Goal

    vs York

Poor passing out of back from Mawson
Smith gives away a poor pass
Could in front of our area
    Goal

    Vs Port Vale
    Nayatanga caught in possession
Didn't defend the subsequent throw in
No one attacks cross, just try to outmuscle
    Goal

Poor pass edge of their area
Pearson chance to tackle or foul in their half (an experienced player would have brought him down)
    Hamilton doesn't track runner
Free shot at the edge of our area
    Goal

I could find more and more but the fact is we are easy to score against. We can't score goals to compensate for this. And very very worryingly we can't fix it. After all look back all season and you will see this pattern and no improvement.

I have to say if the players aren't learning and the coaches can't fix it, there is only one outcome. Be prepared for a new manager or relegation.
